-学生・研修医教育-

従来の授業・実習カリキュラムに加え、医学部4年生の希望者を対象に、2013年より膠原病セミナー (AOT: Autoimmune disease Overview Training) を8月末に開催しております。リウマチ膠原病学に興味を持って頂くきっかけになれば幸いです。

実習中に担当した症例の学会や英語論文での発表も積極的に支援しています。
2018年4月より、当科をローテート中の研修医を中心とした膠原病レジデントカンファレンスを開始しています。全身を幅広く診ることに始まり、標準治療だけでなく最新の知見や患者さんの背景も踏まえての治療選択といった実臨床を学ぶとともに、プレゼンテーション能力を鍛える機会になればと考えています。

-基礎研究-

膠原病・リウマチ性疾患の免疫異常と組織障害を分子レベルで解明し,効率的な治療の開発に貢献したいと考え、現在大学院生と留学生を中心に、精力的に研究を行っています。

  1. 関節リウマチの関節破壊の分子機構の解明、関節滑膜病変部におけるサイトカインの発現異常、病因・病態との関連およびその誘導機構の解明

    様々な因子に着目し、関節リウマチにおける炎症性骨破壊の機序を探索しています。

  2. SLEモデル動物、ノックアウトマウスを用いた自己免疫現象の成立・進展機構解明

    ノックアウトマウスを作成し、SLE発症に関与する因子の生体内での機能を検討しています。

  3. SLE、血管炎などの膠原病の病態形成おけるmiRNAを介したエピジェネティクス的制御機構の解析

    miRNAを網羅的に探索し、膠原病発症における機能を検討しています。

リウマチ・膠原病を対象として、主に観察研究を中心に展開しています。

  1. 抗好中球細胞質抗体(ANCA)関連血管炎の多施設共同前向きコホート研究(RemIT-JAV, RemIT-JAV-RPGN,など)

    公的研究班で行っているANCA関連血管炎を対象とした多施設共同研究です。本研究データを用いて、これまで当科からも多くの研究成果を報告しています。

  2. 関節リウマチに関する観察研究(OKRA1~3)

    岡山大学病院に通院中の関節リウマチ患者さんを対象とした観察研究です。

  3. 全身性エリテマトーデスの多施設レジストリー構築(LUNA2)(昭和大学、長崎大学、横浜市立大学、聖隷浜松病院)

    全国5施設に通院中の全身性エリテマトーデス患者さんを対象として、多種のリサーチクエスチョンに対応可能なレジストリーを構築しています。

  4. 全身性エリテマトーデス診療の質の検討

    多施設共同での全身性エリテマトーデス診療におけるレセプトデータを活用した診療の質指標の開発・検証研究に参加しています。

  5. 皮膚筋炎・多発性筋炎の自己抗体と症状に関する研究

    筋炎特異的な自己抗体と臨床症状との関連を、クラスター解析を用いて分類し、アウトカムとの関連を調べる単施設での歴史的コホート研究です。

  6. 全身性エリテマトーデスの低補体と再燃に関する研究

    全身性エリテマトーデス診療において再燃抑制のために治療介入が必要となる低補体血症を検討するために行った単施設での歴史的コホート研究です。

  7. ループス腎炎の臨床的・組織学的指標に関する研究

    ループス腎炎患者の組織学的活動性病変や慢性病変を予測する臨床的指標を探索するための単施設での歴史的コホート研究です。

  8. リウマチ性多発筋痛症の新たな分類の試み(PRIMERA)

    多様な臨床病態を呈するリウマチ性多発筋痛症患者を、クラスター分類を用いて臨床的な亜分類をすることを試みた単施設での歴史的コホート研究です。

  9. 膠原病患者における糖尿病等の合併症に関する研究(DIMERD)

    膠原病患者における様々な代謝性合併症を網羅的に検討し、合併症の予防や治療への架け橋となるための単施設前向きコホート研究です。
